File an E-Verify Complaint

Use this form to report a business that you think is breaking Ohio's E-Verify laws. E-Verify is a free government program that checks if workers are allowed to work in the United States. Ohio law requires certain construction contractors, subcontractors, and labor brokers to use E-Verify when hiring.

Before You Start

This form takes about 10-15 minutes to complete. Here's what to expect:

  1. Tell us about yourself (or submit anonymously)
  2. Tell us about the business or person you are reporting
  3. Answer a few yes/no questions about what you think happened
  4. Upload any documents that support your complaint (optional)
  5. Review and submit

Helpful info to have ready:

  • The name and address of the business
  • Where the work took place (job site or project name)
  • How you learned about the problem
  • Names of any witnesses (optional)

Don't worry if you don't know the answer to every question. You can select "Unknown" or leave fields blank if they don't apply.
